Fishbone generates breadth. Pareto with data (preferred) or Voting with SMEs (fallback) filters to vital few. 5-Why drills to MIN root cause.
Common deadline
All eight projects target 15 July as the completion date.
Two-question root cause test, plus glossary
A 5-Why has reached a real root cause when both answers are yes:
- Is the cause actionable?
- If actioned, will it prevent or reduce the problem from recurring?
If you stop at lack of training or human error, you have not reached MIN. Ask what specifically is Missing, Incomplete, or Not-followed.
